Great to meet you!

Hi, I’m Belkis Perez, a triple board-certified Psychiatric Mental Health Nurse Practitioner. I am also board-certified in adult-gerontology primary care and acute care. I have more than a decade of experience as a nurse practitioner, with a background in primary care, corrections, acute care, and hospital specialty practice.

I work with teens (18th and over) and adults by telehealth in Florida. People often come to me for anxiety, depression, ADHD, PTSD, insomnia, mood disorders, or substance use. I also care for clients living with chronic medical illness or organ disease, when mental health and physical health need to be treated as one story.

My style is compassionate, direct, and collaborative. I want you to feel heard and to leave each visit knowing what we are doing and why. Care may include a thorough psychiatric evaluation, supportive counseling, practical tools between sessions, and medication when it is a reasonable part of the plan. Medication is never the whole plan. We decide together, review it over time, and change course if it is not a good fit.

What I bring is the ability to sit with complexity without rushing you. I pay attention to your symptoms, your medical history, your other medications, and what “better” would look like in your actual life. If you want a provider who listens carefully, keeps the plan clear, and treats you as more than a diagnosis, I would be glad to work with you.

My approach to therapy

I start by listening. Before we talk about a diagnosis or a prescription, I want a clear picture of what you are carrying—your symptoms, your medical history, your medications, and what you want to change.

Care is collaborative and practical. We name the problem together, set a few specific goals, and build a plan you can actually follow between telehealth visits. That plan may include supportive counseling, skills you can use day to day, coordination with your other clinicians when it is useful, and medication when it is an appropriate option. If medication is part of care, we review why it is being considered, what we will watch for, and when we will reassess. Nothing stays on the list without a reason.

I treat the whole person, not a single symptom. Anxiety, depression, ADHD, trauma, sleep problems, and substance use often sit next to medical illness, family stress, or a major life transition. My background in primary care, acute care, and hospital specialty practice helps me keep psychiatric care aligned with the rest of your health.

You should leave each visit knowing what we decided, what happens next, and how to reach me if something changes. Progress is something we check together: are symptoms easing, is daily life more workable, and does the plan still fit. If it does not, we adjust.

What you can expect from me

We start with a thorough first visit: your psychiatric history, current symptoms, medical conditions, medications, and what “better” would actually mean in your daily life. If organ disease, transplant evaluation, or a long medication list is part of your story, that is built into the plan from day one—not added later as an afterthought.

LOA and Accommodation Paperwork Policy:

I do not complete Leave of Absence (LOA), FMLA, disability, or employer accommodation paperwork for new patients or during the first visit.

These forms are only provided for established patients who have been under my continuous care for a minimum of 6 months.

This policy ensures I can provide accurate and appropriate documentation based on a sufficient treatment history.

Thank you for understanding.
