As a mental health therapist with 15 years of experience, I am pleased to offer administrative services for completing FMLA paperwork, letters for 504 plans, and documentation for emotional support animals. Please note that FMLA paperwork often requires a minimum of 30 minutes to complete due to the need for supporting clinical documentation. The total cost will depend on the complexity and time required for each individual case. These services are separate from EAP or insurance benefits and are priced as follows:
Administrative Services
FMLA Paperwork and 504 Plan Letters
Initial Evaluation: $150 per hour
Subsequent Required Documentation: $75 per hour
Emotional Support Animal Documentation
Requirements:
Minimum of 3 therapy sessions to establish therapeutic rapport
Mental health evaluation to determine necessity of an emotional support animal
Fee: $200 for the evaluation and documentation
This service includes:
Assessment of mental health needs
Evaluation for ESA eligibility
Preparation of official ESA letter
Please note that emotional support animal registration is not legally required in Georgia. The ESA letter provided will be sufficient for housing accommodations under the Fair Housing Act.
These services are designed to support clients' needs while respecting the time and expertise required for such documentation. Fees are due at the time of service and can be paid via the credit card associated with your Headway account.
