Constance “Connie” Stofko, LPC, is a Licensed Professional Counselor and Approved Clinical Supervisor with extensive experience providing compassionate, evidence-based mental health care to individuals and couples. She specializes in working with adults facing anxiety, depression, trauma-related concerns, life transitions, relationship challenges, and co-occurring mental health and substance use disorders.
Connie currently serves as a Licensed Co-Occurring Clinician with a Program of Assertive Community Treatment (PACT) team, providing outreach services and comprehensive support to individuals with serious and persistent mental illness. Her work includes individualized counseling, medication education, symptom management, wellness groups, and collaborative treatment planning within a multidisciplinary team. She has also provided supervision to clinical staff and served as Interim Team Leader, reflecting her strong leadership and clinical judgment.
