Great to meet you!

Hi, I’m Jose Gabriel Chavez, a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) and Adult-Gerontology Acute Care Nurse Practitioner (AGACNP). With dual specialization, I bring a comprehensive medical and psychiatric perspective to patient care.

I specialize in helping adults and adolescents with anxiety, depression, ADHD, trauma, bipolar disorder, and complex medical-psychiatric conditions through thoughtful medication management and supportive therapy. My background in acute care allows me to better understand how physical health issues intersect with mental health.

I’m committed to providing compassionate, personalized care in a safe, non-judgmental space. Whether you’re navigating stress, mood challenges, attention difficulties, or the combined effects of medical and mental health concerns, I work collaboratively with you to create a practical treatment plan that supports your overall well-being.

I look forward to partnering with you on your journey.

My approach to therapy

I take a collaborative, patient-centered approach that combines evidence-based medication management with supportive therapy. As a dual board-certified PMHNP and AGACNP, I address both mental health symptoms and how they interact with physical health conditions.

I believe in creating a warm, non-judgmental space where we work together to understand your goals and develop a personalized plan. My style is practical and empathetic. I listen carefully, explain options clearly, and adjust treatment based on what works best for your life and needs.

What you can expect from me

In our first session, we’ll start with a thorough conversation about your current concerns, history, and goals. I’ll complete a comprehensive psychiatric evaluation and, if appropriate, discuss medication options or adjustments.

You can expect a supportive, confidential environment where I listen without judgment and answer all your questions. By the end of the session, we’ll have a clear plan moving forward so you leave feeling heard, understood, and hopeful about next steps.
