I am a Social Worker on the Telemetry Unit, which is a Cardiac specialty floor. Many of our patients have Congestive Heart Failure, strokes, and other cardiac diseases. I also frequently have patients with dementia at various degrees of severity. I am very interested also in Palliative Care and Geriatrics--my patients often fall into these categories as well.
I work with patients, families, and other agencies to ensure a safe hospital discharge. I also provide supportive counseling around mental health issues, aging, chronic or life-threatening disease, psychosocial stressors, and caring for the elderly. I educate patients and families on entitlements, insurance, and community resources, making referrals when appropriate.
I also serve on the Social Work Education Committee, which holds monthly in-services for our department on relevant topics and new developments within our field.
