I'm Reginald Ibe, a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). I provide telehealth psychiatric care to adults across Texas, Washington, New Mexico, and Nevada — diagnostic evaluation, medication management, and supportive therapy for depression, anxiety, bipolar disorder, ADHD, PTSD, and insomnia.

My approach is collaborative and deliberate. I start low and titrate carefully, because the goal isn't just a lower symptom score — it's a life that feels workable to you. I'd rather have you on two medications that genuinely help than five that each do a little. I'll explain what I'm recommending and why, tell you what side effects to watch for, and expect you to push back if something isn't working. When therapy rather than another prescription is the right next step, I'll say so.

My approach to therapy

I treat medication decisions as shared decisions. I'll tell you what I'm recommending, why I'm recommending it, and what to watch for, and I expect you to tell me when something isn't working. You know how you feel on a medication better than any rating scale does.

I also use screeners at intake and at regular intervals, so we're tracking real change over time rather than relying on impression alone. Early on we'll meet more frequently to get things dialed in, then space visits out as you stabilize.

And I'll be straight with you about the limits of what I do. Medication is one tool. When therapy, a sleep intervention, or a referral is the better next step, I'll say so rather than adding another prescription

What you can expect from me

I'll ask what brought you in and what you're hoping will be different. From there we'll cover your history — symptoms and how long they've been going on, past treatment and what did or didn't help, medical conditions, current medications and supplements, substance use, family history, sleep, and what your daily life actually looks like. Some of it is routine, some of it is personal. You can tell me if a question feels like too much for a first conversation.

You'll complete a few brief screeners before or during the visit. These give us a baseline so we can track real change over time rather than relying on memory.

By the end, I'll share my working impression and we'll talk through options together — what I'd recommend, why, what side effects to watch for, and what the alternatives are. Nothing gets started without your agreement. If I don't have enough information to be confident yet, I'll say that too.

You'll leave with a plan, any prescriptions sent to your pharmacy, and a scheduled follow-up — usually within two to four weeks while we're getting things settled.
