Great to meet you!

I’m Tara Wells, MSN, APRN, PMHNP-BC, a board-certified Psychiatric Mental Health Nurse Practitioner and founder of 316 Psychiatry & Wellness. My background includes specialized training in perinatal mental health, experience in inpatient geriatric psychiatry, community health leadership, and nursing education. I provide outpatient psychiatric care across the lifespan, seeing patients from age 4 to 104.

I provide personalized, evidence-based psychiatric evaluation and medication management for children, adolescents, adults, and older adults. I work with individuals experiencing ADHD, anxiety, depression, bipolar disorder, PTSD, OCD, sleep concerns, dementia and Alzheimer’s disease, as well as mental health concerns related to pregnancy and postpartum, perimenopause, menopause, and other life transitions.

My approach looks beyond symptoms alone. When appropriate, I incorporate sleep, nutrition, laboratory findings, pharmacogenetic testing, and overall wellness into treatment. My goal is to provide thoughtful, collaborative care that considers the whole person and helps each patient find a treatment plan that fits their individual needs.

My approach to therapy

I believe psychiatric care should be collaborative, individualized, and focused on the whole person, not just a diagnosis or list of symptoms. I take time to understand what you’re experiencing, what matters most to you, and how your mental health fits into your overall health and daily life.

My approach combines evidence-based psychiatric treatment with thoughtful consideration of sleep, nutrition, medical conditions, laboratory findings, medications, and other factors that may be affecting how you feel. When appropriate, we may also explore pharmacogenetic testing and other integrative approaches.

Most importantly, I want you to understand your options and feel involved in decisions about your care. Together, we’ll develop a treatment plan that is practical, personalized, and designed around your individual needs and goals.

What you can expect from me

Your first appointment is an opportunity for me to get to know you beyond your symptoms. We’ll talk about what brought you in, your current concerns and goals, medical and psychiatric history, medications, sleep, lifestyle, relationships, and other factors that may be affecting your mental health.

We’ll work together to identify priorities and discuss treatment options, including medication when appropriate. I’ll explain my recommendations, answer your questions, and make sure you understand the reasoning behind your treatment plan.

You don’t need to have everything figured out before you arrive. The goal of your first session is to leave with a better understanding of what may be going on and a clear plan for what comes next.
