Hi, my name is Thomas and I'm a board certified PMHNP. The name of my practice is Addiction Care Plus, LLC; which provides telehealth to those within the entire states of Indiana and Kentucky.

I provide care for those with substance-use disorders; including most comorbid psychiatric conditions. No matter how many times you've tried to overcome your struggles with addiction or mental health; just keep trying- your worth it!

Cash payment visits for General Psychiatry or Substance-Use Disorders are $179/visit.

Cash payment visits for MAT/MOUD (Buprenorphine/Naloxone, naltrexone, etc) are $99/visit.

I provide medication-management and therapy for substance-use disorders and in addition; provide those patients with care of most comorbid psychiatric conditions.

The goal for our first session would be to learn about you, your struggles with SUD/mental health, and your goals of treatment that you are seeking.
