Hi, my name is Thomas and I'm a board certified PMHNP. The name of my practice is Addiction Care Plus, LLC; which provides telehealth to those in the states of Kentucky and Indiana.

I provide medication-management and therapy for those with substance-use disorders; including most of their accompanying psychiatric conditions. No matter how many times you've tried to overcome your struggles with addiction or mental health; just keep trying- your worth it!
