Great to meet you!

I am currently only accepting new clients for initial 15-minute phone/video consultations. If you are a new client, please select a consultation slot. Full intake appointments booked without a prior consultation will be rescheduled.

Welcome! I’m John McCall, a Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) and founder of Cascadia Psychiatry & Wellness LLC in Portland, Oregon.

When mental health feels like a steep climb, having the right guide makes all the difference. I provide comprehensive psychiatric evaluations, ongoing medication management, and collaborative goal-setting for children, adolescents, and adults. As an LGBTQIA+ affirming provider, I cultivate an inclusive, authentic space where all identities feel supported.

We provide compassionate care for a wide range of mental health challenges, including:

  • Sadness & Low Energy: Prolonged low mood, loss of interest, or cycle-related mood shifts.
  • Anxiety & Panic: Overthinking, chronic stress, panic attacks, and social anxiety.
  • Mood Swings: Unpredictable shifts between racing thoughts/high energy and deep lows.
  • Reality Disconnects: Distressing sensory experiences or feelings of detachment.
  • Focus & Neurodivergence: Support for ADHD, restless energy, and autism spectrum styles.
  • Trauma & Stress: Healing from past trauma, distressing memories, or sudden life crises.
  • Life Transitions: Guidance through relationship changes, identity growth, and daily stressors.
  • Medical Coping: Mental health support for chronic illnesses or neurological conditions.
  • Medication Deprescribing: Safe, careful tapering of prescriptions no longer serving you.

My approach to therapy

My Approach to Care

Drawing on a strong background in critical care, my practice focuses on investigating the physiological root causes of mental health symptoms rather than simply treating them at surface level.

I typically:

  • Investigating Root Causes: Thyroid dysfunction, sleep apnea, and blood sugar imbalances directly impact the brain and frequently mimic psychiatric conditions. I dig into your medical history to ensure core physical systems are functioning properly.
  • Targeted Medication Management: Focused on precise prescribing, ongoing monitoring, and supervised deprescribing when medications are no longer necessary.
  • Comprehensive Care: Integrating clinical precision with a collaborative, whole-person approach tailored to your lifestyle and goals.

What you can expect from me

  • Location & Format: I typically see clients in person for our initial comprehensive evaluation at my Sellwood office in Portland to build a grounded connection, following appointments can be virtual.
  • Deep-Dive Evaluation: We will thoroughly review your psychiatric history, current concerns, lifestyle factors, and past medication experiences without feeling rushed.
  • Investigating Root Causes: We will discuss underlying physical health systems to rule out medical contributors.
  • Collaborative Plan: You’ll leave our first visit with a clear, mutually agreed-upon strategy, whether that involves starting treatment, ordering labs, or coordinating care.
