Aloha! I’m Leland Estrella, DNP, PMHNP-BC, FNP-BC, a psychiatric nurse practitioner providing virtual care throughout Hawaiʻi. I’m a Native Hawaiian, born and raised in Waiʻanae, Oʻahu, and I strive to create a warm, respectful, and judgment-free space where patients feel heard, supported, and understood.

I specialize in the evaluation and treatment of ADHD across the lifespan, with a focus on helping children, adolescents, and adults better understand attention, focus, executive functioning, and how symptoms affect daily life, school, work, and relationships. I also provide treatment for anxiety, depression, and autism spectrum disorder (ASD), with an emphasis on practical, individualized care that supports long-term functioning and overall well-being.

As a former active-duty Air Force medic and trauma nurse with three combat deployments, and now an Air Force Reserve officer, I also bring a strong understanding of the unique stressors faced by veterans, service members, first responders, healthcare professionals, and their families.

My approach to therapy

My approach is collaborative, straightforward, and individualized. I believe treatment should be tailored to each person’s needs, goals, and stage of life—not one-size-fits-all. For some patients, medication can be an important part of treatment; for others, the focus may be on education, therapy, lifestyle changes, behavioral strategies, or additional support services.

I work closely with patients to create practical, personalized treatment plans that support daily functioning, emotional well-being, and long-term success. My goal is to provide thoughtful, evidence-based care while helping patients better understand their symptoms and treatment options. When additional services are needed, I’m happy to help connect patients with trusted therapists, testing providers, or other specialists to ensure they receive the right level of support. My practice is committed to safe, evidence-based prescribing and does not routinely prescribe benzodiazepines for long-term treatment.

What you can expect from me

I offer a free 15-minute consultation so we can briefly discuss your concerns, what you’re looking for in treatment, and whether my practice feels like a good fit for your needs. If we decide to move forward, we’ll schedule a full psychiatric evaluation.

The initial evaluation is a more comprehensive visit where we review your current concerns, symptoms, history, strengths, and goals to develop a personalized treatment plan together. Initial evaluations are typically 60 minutes for adults and 90 minutes for children and adolescents to allow time for a thorough assessment, questions, and collaborative treatment planning.

Follow-up visits are generally 30 minutes for adults and 30 to 60 minutes for children and adolescents, depending on clinical needs, treatment complexity, and whether additional parent or family discussion is needed. My goal is for patients and families to leave each visit feeling heard, informed, and clear about the next steps.
