Welcome! I am Jeremy Wade, a dual board-certified Family and Psychiatric-Mental Health Nurse Practitioner with over two decades of healthcare experience. After spending 25 years as an ER provider and clinical leader, I saw too many patients hospitalized simply because they couldn't access proactive outpatient psychiatric care. Driven by a passion to help this underserved population, I stepped away from the ER to dedicate myself fully to mental wellness. Today, I combine my deep medical background with my growing mental health expertise to treat the "whole you" in a safe, welcoming, and collaborative environment.

My approach to therapy

Because of my extensive emergency and family medicine background, I deeply understand the complex ways that your physical and mental health intersect. My transition into full-time mental health has taken me across outpatient clinics, acute inpatient units, and residential treatment facilities, allowing me to learn from highly skilled physician mentors and weave the best of those tried-and-true methods into my practice. I am highly pragmatic: I do not chase trends, and I only recommend treatments based on evidence and what will actually work best for you.

I treat patients across the lifespan, from adolescents to older adults. My specialized clinical focuses include:

  • Acute psychiatric stabilization
  • Substance use and withdrawal (utilizing my SAMHSA Buprenorphine Waiver)
  • Complex medical/psychiatric overlap (safely managing psychiatric medications for patients who also have chronic physical health conditions)

During our first visit, my primary goal is to understand your unique story, your current struggles, and your specific goals. You can expect a thorough psychiatric evaluation in a collaborative, judgment-free environment. We will look at the "whole picture"—including your physical health, lifestyle, and past treatments—to figure out what is working and what isn't.

By the end of our first session, we will work together as a team to develop a personalized treatment plan tailored specifically to your life and your body. Whether that involves evidence-based medication management, brief psychotherapy, or a combination of both, my ultimate goal is to provide the accessible, high-quality outpatient care necessary to keep you safe, stable, and thriving at home.
