Jacob Esses

Hello! My name is Jacob Esses and I am a board-certified Psychiatric–Mental Health Nurse Practitioner (PMHNP-BC) and earned my graduate degree from Stony Brook University. I have over four years of experience as a psychiatric nurse, with a background in inpatient, emergency, and outpatient mental health settings. I work with adults and provide evidence-based, patient-centered psychiatric care. I strive to create a respectful, non-judgmental environment where clients feel heard, supported, and involved in their treatment.

I take a collaborative and individualized approach to care, believing that effective treatment begins with understanding each person’s unique experiences and goals. I develop personalized treatment plans that may include medication management, education, lifestyle strategies, and therapeutic guidance when appropriate. My style is supportive, transparent, and grounded in evidence-based practice. I prioritize shared decision-making so clients feel informed and empowered throughout the treatment process.

In the first session, I conduct a comprehensive psychiatric evaluation in a calm and supportive environment. I take time to review symptoms, psychiatric and medical history, and personal goals while allowing space for clients to share what matters most to them. Together, we begin to outline a clear and realistic treatment plan. Many clients leave the session with a better understanding of their symptoms, treatment options, and next steps.
