Nkem Chukwu

Nkem Chukwu is a certified Family Nurse Practitioner (FNP-C) and a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). She provides assessment, diagnostic, and medication management for all patients across the spectrum. She has worked in a variety of environments both in and outpatient settings with patients ranging from newborns to geriatrics.

She prides herself on her communication skills by tailoring an interprofessional model of care specialized for each patient to achieve a stable physical, mental, psychiatric, and emotional wellbeing.

My goal is to utilize my knowledge and practical skills acquired through education and clinical rotation in planning patients care. My passion as a healthcare provider center primarily on maximizing available healthcare resources in providing timely, quality, cost effective and result-oriented care to my patients.
