Welcome! I am Jeremy Wade, a dual board-certified Family and Psychiatric-Mental Health Nurse Practitioner with over two decades of healthcare experience. After spending 25 years as an ER provider and clinical leader, I saw too many patients hospitalized simply because they couldn't access proactive outpatient psychiatric care. Driven by a passion to help this underserved population, I stepped away from the ER to dedicate myself fully to mental wellness. Today, I combine my deep medical background with my growing mental health expertise to treat the "whole you" in a safe, welcoming, and collaborative environment.

With decades of experience as a nurse practitioner and over 15 years specializing in mental health, I am committed to understanding each patient's unique story and life experiences. I have a special interest in resiliency and thriving after trauma. I am dedicated to supporting you as you determine the path and pace that best suits your needs.

While I've worked across many demographics, my practice now focuses primarily on adult individuals navigating anxiety, relationship and intimacy concerns, trauma, and major life or identity transitions. I'd be glad to help you find steadier ground, whatever that looks like for you.

I have spent most of my career working with children and families in community mental health, but I am now focusing on providing telehealth services for adults. My background includes extensive experience supporting individuals with trauma, anxiety, depression, ADHD, and executive functioning challenges. I am passionate about helping people navigate life transitions and overcome obstacles, and I strive to create a supportive and empowering environment for every client.

I am a Licensed Professional Counselor with the Oregon State Board. I completed my masters degree at Adams State University in Colorado.

I am a Licensed Marriage and Family Therapist with a Master’s degree in Couples and Family Therapy. I have extensive experience working with children, teens, adults, and families across a range of concerns. My specialties include anxiety disorders, trauma and PTSD, ADHD and executive functioning challenges, and child and adolescent therapy with parent coaching. I enjoy helping clients better understand themselves, navigate difficult emotions, and build healthier relationships and coping strategies.
